By completing this form, your school is committing to participate in the Highmark Healthy High 5 T.R.U.E. Cards...Life Lessons project. You also agree to complete up to three evaluation surveys or interviews during the course of two academic years.

Each school or organization should only enroll one time. The school principal or designated lead teacher should complete the following enrollment form.  This person will be contacted and requested to coordinate survey participation with all persons using T.R.U.E. Cards.
